Osteoporosis is defined as a pathological course in bone tissue that leads to its reduction and thinning.  During this process, the bone structure is destroyed at a microscopic level, resulting in increased bone fragility. More recently, it has been discovered that there is a connection between the appearance of osteoporosis and the condition of the teeth and jaw.

Osteoporosis symptoms

Osteoporosis usually develops because there is a deficiency of calcium, which is the main building block of bones and teeth.
When the bone density of the jaw tissue decreases, various dental pathologies develop, which contribute to the destruction of teeth, followed by their removal. Prosthetics are needed to restore the functionality and aesthetics of the jaw system. Many patients wonder if they can have dental implants when they have osteoporosis. There is no direct contraindication to the procedure in this case. Some doctors, on the contrary, recommend the implantation of titanium roots, as it triggers the regeneration processes and the bone becomes denser.

Indications and contraindications 

With implants, it is possible to restore or replace a destroyed tooth that must be extracted, rebuild a tooth segment or a complete row in cases of multiple adentia. It is performed on the upper jaw and the lower jaw.
If the patient has a history of osteoporosis, the decision about implantation is made by the doctor after careful diagnosis. This is done with a 3D computer tomography scan. On this scan, the condition of the jawbone can be seen at any angle, and layer by layer. On the volumetric image, the doctor evaluates the degree of disease development, bone atrophy, mineral composition. 

The list of contraindications includes:

  • tuberculosis;
  • clotting disorders, blood diseases;
  • malignant tumors;
  • bruxism;
  • mental disorders;
  • severe vascular and cardiac abnormalities;
  • sexually transmitted diseases;
  • abnormal bite;
  • acute infections;
  • exacerbation of chronic diseases;
  • alcoholism, smoking, in which the patient cannot give up cigarettes for a long period of time;
  • pregnancy, lactation;
  • exacerbation of chronic diseases;
  • low level of immunity.

Some contraindications are of temporary nature and surgery becomes possible after their elimination.

Implantation protocols for osteoporosis

The one-step implantation with immediate loading required for dental implants for osteoporosis includes several protocols, all-on-4 or all-on-6, to restore lost teeth. Methods of implantation

Depending on the clinical picture, implants can be implanted according to two-step or one-step treatment protocols. The latter is preferable, because the screws can be inserted at an angle, which provides greater bone coverage and therefore the system will be better stabilized.
In the classic technique, the implant is placed first and the prosthetic is implanted after an average of six months. The surgery involves gum incision and stitching, which means that it is highly traumatic. Bone augmentation is practically always performed. Bone grafting also requires a recovery period. 
One-step treatment protocols allow you to restore your teeth in one step, without blood or sutures, because micro punctures are made in the gum to introduce the artificial root. The minimally invasive procedure is more comfortable for the patient and the recovery period is easier.

Methods of single-stage implantation:

  • Implant placement immediately after tooth extraction - one-stage;
  • Restoration of a tooth segment or a full row - basal;
  • Treatment of full adentia - All-on-4 or All-on-6 protocols.
  • The doctor decides which method to choose after diagnosis.

Stages of implantation

Regardless of the treatment protocols, the procedure is performed sequentially. The classic method of correction is very time-consuming, and all manipulations by the one-phase method can take only a few days. This period includes diagnosis, surgery to install the titanium roots, and prosthetics. 

Preparation for implant placement

At the initial appointment, the doctor collects medical history, examines the oral cavity, and conducts a CT scan. The patient undergoes blood and urine tests to rule out general contraindications. A general practitioner's authorization may be required.
If one or more implants are placed, professional oral hygiene cleaning of plaque is performed. If dental diseases (caries, gingivitis, etc.) are identified, appropriate treatment is performed first.
After diagnosis, the dentist chooses the method of restoring the dentition and the implant system. If full adentia is treated using one-step protocols, 3D modeling of the result is performed, and surgical templates are printed.

Performing the implantation

The procedure is performed in stages:

  1. anesthesia with local anesthetics;
  2. Preparation of the bed for the titanium root;
  3. screwing the implant into the bone.

If the tooth is extracted beforehand, the prosthesis is inserted the same day. In this case, special systems are used that can be inserted directly into the hole.

The final step is prosthetics

In the immediate denture protocol, temporary crowns are placed within three days. Permanent dentures are placed after a minimum of six months. Some temporary prosthetic systems may last up to several years.
In case of delayed loading, the prosthetic restoration is made after the implant has fully taken root. In this case, a permanent crown is placed immediately.

Rehabilitation period after implantation with osteoporosis

he doctor's prescription must be strictly adhered to. If antibiotics are prescribed, you must take the recommended course. Pain can be relieved with analgesics, swelling - with a cold compress.
In the first two weeks it is important to take good hygienic care of the mouth. You need to carefully remove plaque with a soft toothbrush, floss and rinse. In order not to disturb the traumatized tissues, you should eat soft food at a comfortable temperature. It is not recommended to go to the bathhouse, take a hot bath, sunbathing on the beach.

Smoking should be avoided for the whole recovery period, until the process of osseointegration is completed, about 4-6 months.

Prevention of osteoporosis in dentistry

If concerns about osteoporosis are confirmed, but are not yet critical, you should definitely see a doctor and follow the preventive recommendations. The therapeutic and preventive measures for bone depletion initially aim to saturate the body with such vitamins and minerals that will contribute to the process of regeneration of cartilage and bone tissue. And this means that the most important place in such prevention is the diet, so the first thing doctors recommend changing the diet:

Forbidden foods: too spicy, fried foods, foods with high acid content (radish, sorrel, etc.), salty snacks, vinegar pickles, canned foods, carbonated water, coffee, alcoholic beverages.

Recommended foods: soups, fish and meat (lean), seafood, vegetables, cereals, pasta, dairy and milk products, pastries (yesterday or dried), berries, dried fruits, eggs, plain, herbal and fruit teas, vegetable and animal fats.

You should not forget about oral hygiene, and brush your teeth after each meal. It's no secret that normal plaque hardens and turns into tartar, which in turn activates the inflammation of periodontal tissues. Hence, periodontitis.
